Your new role
As the Case Manager, you will be expected to co-ordinate and oversee home care services for clients with government-funded Home Care Packages. This includes assessing needs, developing care plans, managing service delivery, and ensuring high-quality care that supports client independence and well-being. The Case Manager is the main contact for clients and families, helping navigate services and addressing care-related issues.
Your key responsibilities will be to:
- Conduct comprehensive assessments and develop individualised care plans.
- Regularly review and update care plans based on client needs.
- Coordinate home care services, liaising with providers and caregivers.
- Monitor and adjust services to ensure client satisfaction.
- Act as the primary contact for clients and families, providing support and advocacy.
- Assist with navigating the aged care system and financial enquiries.
- Manage Home Care Package budgets, ensuring cost-effective service delivery.
- Provide budget reports and guidance to clients and families.
- Maintain accurate documentation and address compliance issues promptly.
- Work with multidisciplinary teams to provide holistic care.
- Maintain regular communication with clients, families, and service providers.
- Handle feedback and complaints professionally and promptly.
- Use feedback to improve care delivery and client satisfaction.
- Participate in initiatives to enhance care quality and efficiency.
- Stay updated on legislation, guidelines, and best practices in the industry.
What you'll need to succeed
To succeed in this role, you will need to possess the following:
- The relevant tertiary qualifications or previous experience in the same or similar role.
- A strong background and knowledge of NDIS policies and procedures.
- The ability to be empathetic, patient and dedicated to improving the lives of your participants.
- Strong organisational skills.
- Be a team player and a good communicator.
- Strong assessment and care planning with a person-centred approach.
